This is the place to do your life's best work.(sm) As a Special Needs Basic Care (SNBC) Care Coordinator you will be responsible for case management and care coordination for members with mental health and substance use disorder conditions.
You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan.
Primary Responsibilities: Management and authorization of behavioral health services, including crisis assessment and intervention Integrates a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services to meet the enrollee's health needs, using education, communication, and all available resources to promote quality, cost-effective outcomes Completes an Assessment on each member as well as an individualized Care Plan to address the member's needs Telephonic and field-based visits (members' homes) with members to determine/coordinate treatment or resources to address the member's complex needs Discharge planning or management of transitions between care settings to ensure the appropriate services/resources are in place for quality outcomes Management of behavioral and medical care coordination for all complexity levels Make outreach calls to members and/or providers for coordination of behavioral and medical services Assist consumers by screening, answering questions, providing program information and education about care coordination program services, network and community resources, and coverage determinations in an accurate and courteous manner Required Qualifications: Master's or Doctoral Degree in Psychology, Social Work, Counseling or related field, or a Bachelor's in Nursing Current, non-restricted independent Minnesota license in a behavioral health field or RN licensure 2+ years post-licensure clinical experience in an appropriate behavioral health environment Strong proficiency in MS Office Suite Must be able to accommodate daily field-based travel within the determined service delivery area (Greater Minnesota) Preferred Qualifications: Experience in working in managed care environment Substance abuse experience Medical or behavioral health collaboration within different organizations Working with SPMI population Experience providing care coordination Excellent Customer Service Skills Excellent Organizational skills Discover a diversified health services and innovation company where data, technology, people and action combine to make great things happen every day - and where every day is another opportunity to do your life's best work.(sm) Diversity creates a healthier atmosphere: UnitedHealth Group is an Equal Employment Opportunity/Affirmative Action employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, age, national origin, protected veteran status, disability status, sexual orientation, gender identity or expression, marital status, genetic information, or any other characteristic protected by law.UnitedHealth Group is a drug-free workplace.
